From home to nursing home From Home to Nursing Home is a product for relatives that communicates and organizes the vast amount of information provided during the transition of a dementia patient from their home to a nursing home. In connection with this, research was conducted to explore the significance of different visual expressions on the emotions and feelings experienced during this period. The result of this work is a welcome package designed to make the transition from home to nursing home more manageable and easier for relatives of dementia patients.

The welcome package functions as a folder containing 6 pamphlets, each with its own set of information. This division allows individuals to select or deselect information based on their specific needs.

The welcome package also includes a feature that enables relatives to customize it further. The ring binder mechanism and format encourages the insertion of personal documents at the back of the folder, with a ribbon providing easy access to this section. Visually, a calm color gradient has been chosen, symbolizing the progression of the disease.
